Dr. Joel Mallari, DNP, AGPCNP-BC

Doctor of Nursing Practice (DNP) Board Certified (BC) as Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP) with intensive experience in critical care at UC Davis Medical Center and others. Dr. Mallari provides on-call services for the practice.

Angela Wilson (Office Manager)

Handles general inquiries, refills, lab/diagnostic requisitions, and care coordination.  Ms. Wilson has a Bachelor of Science in Nursing Degree.

Dr. Frederick Arredondo, MD

Dr. Arredondo did 19 years of house call practice in New York (2000-2019) before moving back to California.  He obtained his Doctor of Medicine (MD) degree with the State University of New York Upstate Medical Center in Syracuse, NY, and residency at Highland General Hospital in Oakland, CA. He is Board Certified in Family Practice with AAPS Board Certification and has special focus and interest in wound mamagement. 

Dr. Ron Billano Ordona, DNP, FNP-BC, GS-C

Doctor of Nursing Practice (DNP) ​Board Certified (BC) in Gerontology and as a Family Nurse Practitioner (FNP) and Certified Gerontological Specialist (GS-C). More than fifteen years of experience in healthcare including rotations at UC Davis Medical Center, and others. Dr. Ordona is a Health Sciences Assistant Clinical Professor at the UCD Healthy Aging Clinic. Dr. Ordona's focus is on community-based residential care.  

Anquinette Cray, MSN, FNP-C

Masters of Science in Nursing (MSN) Certified (C) Family Nurse Practitioner (FNP) working on her doctorate with intensive experience in critical care at UC Davis Medical Center. Ms. Cray provides on-call services for the practice.

Services

Christine Henson, BSN, RN (Practice Manager)

Handles intake, insurance, billing, and referrals. Ms. Henson is a Registered Nurse (RN) and has a Bachelor of Science in Nursing (BSN) Degree, and manages both Lincoln Medical Practice and Senior Care Clinic House Calls.

Geriatrics Medical Care: We review and reconcile medications; we prescribe and refill medications according to the plan of care; we co-manage with other members of the healthcare team (e.g. home health nurses, physical therapists, occupational therapists, social workers, pharmacists, and others) for ongoing state of health of the client.

Transitional Care Management (TCM) House Calls: Medicare-covered one time-visit after discharge from the hospital or skilled nursing facility (SNF).
Annual or As-Needed Physicals: where going to the doctor's office is a challenge (e.g. dementia; debilitated; bedridden; or simply no family member or responsible person available). We will do the physical examination at the client's own home or place of residence (e.g. Retirement Home, Independent Living, Assisted Living, Memory Care, or Board & Care Home)
Full Laboratory Testing: we arrange for laboratory testing without the client leaving home. We arrange for specimen collection (e.g. blood draws, catheterization, and the like). We arrange for X-ray or ultrasound exams done in the home. Results are reviewed by the medical provider. Some services may not be covered by Medicare.
Adult Immunizations: such as Flu vaccine, pneumonia vaccine, and others as indicated, done in the patient's residence.
Preventive Health Care: ongoing visits to monitor responses to medications and other therapies; evaluate home safety; evaluate the level of care, and the like. The regular visits rotate on a 3 or 6-month cycle depending on program (please ask us for details).
LIC602 (Physician's Report) w/wo TB Testing, and other requirements for facilities, accomplished where the client is located and in shorter turnaround time (3 days if including TB test and reading), no need to leave home.

Chronic Care Management: A Medicare-covered benefit for members who have two or more chronic conditions.

Telehealth Visit: As allowed by current regulations, some visits may be done via Telehealth (Video) visits.
