Services
DISCLOSURE: Out-of-pocket fees are for services not covered by insurance. We do not offer concierge medical services at this time. Please allow 24-72 hour turnaround time for responses. This service is not intended to replace activating the Emergency Medical Services (EMS). In the case of a medical or psychiatric emergency, please go to the nearest emergency department or dial 9-1-1.


Service options*


Option 1 Medical House Call Program 
Cost: $100 per month, 6 months paid in advance. Covers cost not covered by Medicare. Renewable every 5th month, no long-term commitment, non-payment cancels the participation. Please call us for more details.

Option 2 Single visits, as needed. Non-emergency visits.
Visits are done between 7 AM to 7 PM; other hours are based on need and availability of providers. This option includes Lic602/Physicians Report forms. The cost to the patient: $350 per visit. This visit is cash/private pay and not billed to Medicare or Insurance. Faster service.

Option 3 Primary Care Billed to Medicare (non-HMO)
The cost to the patient: co-pay/deductible, if applicable.
Regular Clinic Hours (Not a house call visit). 
Patients are seen in a regular clinic setting.

Option 4 Transitional Care Medical House Call, billed to Medicare (non-HMO)
The cost to the patient: co-pay/deductible, if applicable.
Medical House Call Visit within 7 days of discharge from hospital or SNF.

The goal is to reduce unplanned readmission to ER/hospital.

We cover the refills for up to 3 months, to give time for patients to re-establish with own Primary Care Provider (PCP).


Option 5 Chronic Care Management (CCM), billed to Medicare (non-HMO)
The cost to the patient: co-pay/deductible, if applicable.
This option is mainly non-face-to-face care. Medical House Call Visit (face-to-face) based on the availability of provider/s and can take from 3 days to 1 week for a visit, or every 3-6 months (routinely), whichever comes first.
Medicare requires a contract signed to cover for this type of service.

Misc charges*
$50 TB test (charges for TB test serum only), $195 if RN to administer and read (2 visits)
$50 Immunizations (charges for vaccine only), $135 with RN to administer
$150 for employee physicals (includes TB tests and reading)
$350 for Lic 602/Physicians Report with TB test (2 visits)
$85 RN/LVN visits (1 hr or less):  lab draws, catheter care, tracheostomy care, injections and other procedures.


*Some services may not be covered by Medicare. Medicare requires an Advanced Beneficiary Notice (ABN) form CMS-R-131 to be signed by the patient or responsible person. Rates are subject to change without prior notice. Please call us for details.

Services Explained

  • Geriatrics Medical Care: We review and reconcile medications; we prescribe and refill medications according to the plan of care; we co-manage with other members of the healthcare team (e.g. home health) for ongoing state of health of the client.
  • Annual Physicals: where going to the doctor's office is a challenge (e.g. dementia; debilitated; bedridden; or simply no family member or responsible person available). We will do the physical examination at the client's own home or place of residence (e.g. Retirement Home, Independent Living, Assisted Living, Memory Care, or Board & Care Home)
  • Full Laboratory Testing: we arrange for laboratory testing without the client leaving home. We arrange for specimen collection (e.g. blood draw, catheterization, and the like). We arrange for X-ray or ultrasound exams done in the home. Results are reviewed by the medical provider. Some services may not be covered by Medicare.
  • Adult Immunizations: such as Flu vaccine, Pneumonia vaccine and others as indicated, done in the patient's residence.
  • Preventive Health Care: on going visits to monitor responses to medications and other therapies; evaluate home safety; evaluate level of care, and the like.
  • LIC602 (Physician's Report), TB Testing, and other requirements for facilities, accomplished where the client is located and in shorter turnaround time (3 days if including TB test and reading), no need to leave home.
