Senior Care Clinic House Calls provides medical house calls for homebound seniors. Our goal is to achieve your highest quality of life at your current level of function. All new patients are assessed for eligibility or care preference. Our support team will assess your eligibility for care and discuss your care preference with you. We look forward to setting up your first care visit!

Geriatrics Medical Care
Our support team specializes in providing care to our home-bound seniors. We look at your unique needs and create a custom plan of care for those needs so you can thrive.
Medication Management
Our support team will review and reconcile all of your medications. We prescribe and refill medications according to your plan of care.
To provide you with the best care possible, our team will co-manage your care with other members of your healthcare team such as home health nurses, physical therapists, occupational therapists, social workers, pharmacists, and others. We care for your ongoing state of health.

We offer an extensive array of services so you can be the best version of yourself you can be.
Transitional Care Management (TCM) House Calls
TCM House Calls is a Medicare-covered one-time visit after discharge from a hospital or skilled nursing facility (SNF).
Annual or As-Needed Physicals
Is getting to the doctor’s office a challenge? No problem! We will do the physical examination at your own home or place of residence.
Chronic Care Management
We provide chronic care management, which is a Medicare-covered benefit, for members who have two or more chronic conditions.
Full Laboratory Testing
We will arrange for laboratory testing without you having to leave your home. We will arrange for specimen collection (e.g. blood draws, catheterization, and the like). We can also arrange for X-ray or ultrasound exams done in yourur home. Results are reviewed by your medical provider. Please note: some services may not be covered by Medicare.
Adult Immunizations
We are able to provide you with the flu vaccine, pneumonia vaccine, and others as indicated, in the comforts of home.
Preventative Health Care
This includes ongoing visits to monitor your response to medications and other therapies; evaluate your home safety; evaluate your level of care, and the like. These regular visits rotate on a 3 or 6-month cycle depending on your program. Please ask us for details.
LIC602 (Physician’s Report) w/wo TB Testing
and other requirements for facilities, accomplished where you are located and in a shorter turnaround time (3 days if including TB test and reading), with no need to leave the comforts of home.
Telehealth Visits
Telehealth visits are available according to current regulations or as arranged by you, your family, or your DPOA.
